This booklet provides a place where a patient can gather important information they need throughout diagnosis, treatment, follow-up care and long-term management of a blood cancer, such as leukemia (ALL, AML, CLL, CML), lymphoma (NHL, HL), or multiple myeloma. It discusses having a survivorship care plan and includes a list of questions to ask the healthcare team. It provides information on long-term and late effects (cognitive effects, physical effects, psychological effects, possible long-term and late effects of cancer treatment), side effect management, educating family members and friends about long-term and late effects, healthy behavior recommendations (other disease screenings, immunizations), and mental health (fear of relapse, staying calm strategy, creating a self-care plan, body image and self esteem, managing depression). It also discusses relationships and family, support for a caregiver, dating and new relationships, sex and intimacy, and planning for the future.
